Qualifications

• AA/BA degree required; licensure preferred.
• Experience with SPMI and/or SUD populations preferred.
• Strong interpersonal and crisis intervention skills.

Responsibilities

• Provide community-based services to support patients with severe and persistent mental illness.
• Develop and connect patients to resources such as housing, healthcare, and employment.
• Advocate for patients’ rights and needs.
• Promote healthy lifestyle choices addressing physical and mental health.
• Offer crisis intervention in emergencies to de-escalate and ensure safety.
• Collaborate with the interdisciplinary team on individualized treatment plans.
• Monitor progress and adjust plans to achieve goals.
• Maintain monthly productivity and up-to-date records of progress and plans.
