Job summary

Qualifications

• Bachelor's degree in human/social service field, or associate degree with equivalent field experience.
• 3 years working with SMD population.
• First Aid; CPR; De-escalation.
• Valid Ohio driver’s license; proof of automobile liability insurance; meets OMHAS licensure and certification requirements; competencies for serving the SMD population.

Responsibilities

• Provide individual case management under clinical supervision, meeting productivity and quality requirements.
• Meet or exceed standards pertaining to services provided.
• Accumulate the required number of billable hours of clinically allocated time.
• Ensure ACT Case Manager-canceled appointment rate does not exceed 10%.
• Meet or exceed standards pertaining to assessments and documentation.
• Identify service plans and provide linkage to these services within authorized duration.
• Ensure progress notes reflect client progress and actions.
• Complete progress notes within 24 hours of completion.
• Maintain quality clinical charts for all clients.
