Job summary

Bloomington Medical Services in Wooster, OH is seeking an ACO Care Coordinator. This full-time, hourly role supports care planning and navigation for high-need patients under the guidance of the Value Based Specialist and VBS team.

The candidate must be a Registered Nurse (ADN) with at least one year of related experience, possess excellent communication and organizational skills, and be proficient with Microsoft Office, especially Excel.

Job Title: ACO Care Coordinator Department: Administration

Reports to: Amanda Strouse FLSA Status: Non-Exempt ( Hourly)

Job Summary:

Under administrative direction of Value Based Specialist (VBS) in collaboration with Bloomington practices helps patients understand treatment plans and navigate resources.

Duties/Responsibilities:

• Help High need patients understand treatment plans and following provider instructions.
• Care coordination with patients to navigate medical care and non-medical need resources for example food, transportation, housing.
• Assists patients with navigation of transition of care from inpatient to ambulatory and/or follow up appointments from emergency department visits.
• Assists VBS in closing care gaps, scheduling and tracking of annual wellness visits
• Assists Practices in chronic care management
• Communication between providers/patients/VBS/ACO team

Required Skills/Abilities:
