Job summary

Bloomington Medical Services in Wooster, OH is seeking an ACO Care Coordinator. This full-time, hourly role supports care planning and navigation for high-need patients under the guidance of the Value Based Specialist and VBS team.

Qualifications

Responsibilities

• Help high-need patients understand treatment plans and provider instructions.
• Coordinate care and navigate resources for medical and social needs.
• Assist navigation of care transitions from inpatient to ambulatory care.
• Support closing care gaps and scheduling annual wellness visits.
• Assist with chronic care management in collaboration with VBS/ACO team.
• Communicate effectively with providers, patients, VBS and care teams.
