Job summary

Holton Manor in Wisconsin is seeking a CNA to provide patient/resident care under the supervision of licensed nurses. The CNA will complete assigned tasks to ensure timely, safe care and will interact with patients, residents, families, and the health team.

This on-site position offers a flexible schedule with opportunities for tuition reimbursement, EAP, and competitive benefits. Prior hospital or long-term care experience and CPR certification are preferred.

Qualifications

• Must be in good standing on the State Nursing Assistant registry or equivalent
• English literate with excellent written and verbal communication skills
• CPR certification is preferred

Responsibilities

• Performs initial and ongoing data collection under RN/LPN supervision
• Implements care plan developed by RN based on patient needs
• Provides direct patient care and reports changes to RN/LPN
• Demonstrates time management and prioritization of tasks
• Documents care results per policy
• Ensures nutritional care and communicates with residents and families
• Completes other duties as assigned

Job Summary:

TheCertified Nursing Assistant (CNA) will provide patient/resident care as required under the direct supervision of the licensed nurse.TheCNA at Holton Manorcompletes assigned tasks to ensuretimely,efficientandsafe patient/resident care. CNAbase hourly wage:$19.00 – $22.28per hourdependenton experience

PLUSshift differentials- $2.00 per hour PMshiftand $2.50 per hour NOC shift

Key Responsibilities:

• Performs initial and ongoing data collection on all patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.
• Implementsplan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural,ethnicand religious/spiritual needs.
• Provides direct patient/resident care andreportscondition and changes to RN or LPN.
• Demonstrates time management and prioritization of tasks.
• Document results of patient/resident care within established policies,proceduresand scope of practice.
• Nutritional care
• Effective communication with residents and family
• Completes all other duties as assigned.

Qualifications:

• Required:

• Ability to meet physical and job-related requirements
• Must be English literate andpossessexcellent writtenandverbal communication skills
• Preferred:
• Previoushospital or long-termcare or skilled nursing experience
• CPR Certified
