Job summary

Advocate Health is seeking a Patient Logistics RN to join the Mobile Medicine - Care Coordination Center. This full-time role supports patient transfers during 7p–7a with a 36-hour work week, coordinating urgent and complex cases across facilities.

You will perform clinical assessments, document in EMR, collaborate with physicians and transport teams, and participate in quality and safety initiatives to improve transfer processes and patient outcomes.

Qualifications

• ADN degree required.
• Licensed RN in the relevant state.
• Minimum 3 years nursing experience.
• BSN preferred.
• Certified Acute Care Virtual Registered Nurse (CAVRN) preferred.

Responsibilities

• Coordinate safe patient transfers between facilities and internal departments.
• Perform triage assessments to determine level of care.
• Serve as liaison between referring and receiving facilities with clear communication.
• Collaborate with interdisciplinary teams for patient-centered care.
• Document all transfer activities in the EMR and transfer center software.
• Contribute to quality and safety improvement in transfer processes.
