Scripps Health is seeking a Part-time Central Transfer Center RN Care Manager to coordinate safe patient transfers within the system. The role emphasizes strong clinical judgment, collaboration with physicians and care teams, and ensuring appropriate facility placement and level of care while supporting patient flow.

You will screen patients in your caseload for medical vulnerability, coordinate care with the health team and family, and develop transition plans based on patient needs.

Qualifications

• Screen patients in assigned caseload to identify those medically vulnerable.
• Coordinate with health care team to address issues impacting care.
• Develop a transition plan of care based on patient needs.

Responsabilités

• Coordinate safe, efficient patient transfers across the Scripps Health system.
• Liaise with referring providers, internal departments, and receiving units.
• Support patient flow and timely access to services.
