Job summary

Qualifications

Responsibilities

• Coordinate patient transfers for emergent, urgent, or complex cases with clinicians and transport staff.
• Perform triage assessments to determine level of care and subspecialty needs.
• Serve as liaison between referring and receiving facilities, documenting transfer requests.
• Collaborate with interdisciplinary teams to ensure patient-centered care and escalation when needed.
• Document transfer activities in EMR and transfer center software; produce monthly reports.
• Participate in quality improvement to enhance transfer processes and patient outcomes.
