Job summary

Legacy Concierge is seeking a dedicated Caregiver to provide 1:1 non-medical home-based care, typically for an 8–12 hour shift. The role emphasizes personalized, patient-centered support and continuity of care in clients’ homes.

Ideal candidates have 5+ years of caregiving experience, hold a CA Registered Home Care Aide, and can manage ADLs/IADLs while maintaining HIPAA-compliant documentation and compassionate communication with clients and families.

Qualifications

• 5+ years of professional caregiving experience or equivalent education/clinical hands-on experience.
• Registered Home Care Aide in good standing (CA only).
• Proof of negative tuberculosis test/chest x-ray.
• Ability to work independently and follow instructions with questions when clarification is needed.
• Strong communication, documentation, professionalism, and customer service skills.

Responsibilities

• Deliver 1:1 personalized non-medical care in the client’s home.
• Assist activities of daily living (ADLs) such as personal care, transfers, bathing, dressing, incontinence care, medication reminders, hand hygiene.
• Assist instrumental activities of daily living (IADLs) such as meal prep, light housekeeping, transportation, companion care.
• Complete daily care notes accurately and timely using agency systems.
• Manage care for clients with conditions like Alzheimer’s, dementia, Parkinson’s, post-operative recovery, MS, memory loss, COPD, CHF.
• Communicate changes in client status to RN Care Ambassador or Client Care Coordination team.
• Maintain reliable schedule and punctuality; update management on scheduling changes.
