Job summary

Adoration Health is seeking a dedicated Home Health LPN to join our Wayne, NJ team on a full-time basis. You will provide 1:1 home health care, following the patient’s plan of care, and work with a supportive team to ensure quality outcomes.

Qualifications

• CPR certification required.
• 1+ years nursing experience preferred.
• LPN/LPT/LVN degree with current NJ license.
• Valid driver’s license and proof of car insurance.

Responsibilities

• Provide 1:1 care according to the client’s plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate or order supplies, medications, and equipment as needed.
• Ensure patient needs are met per physician instructions.
