Qualifications
- Graduate of an accredited practical nursing program and active LPN license in NJ.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing with strong communication and compassion.
Responsibilities
- Provide home-based care to patients under RN supervision.
- Work with physicians, RNs, CNAs to deliver quality patient care.
- Take vitals, assess, document progress, and report changes to RN supervisor.
- Assist in carrying out patient plans of care and teach self-care techniques.
- Educate family on plan of care, medications, and home safety.
- Collaborate with interdisciplinary team to coordinate care and discharge planning.
