Job summary

Adoration Health in Westminster, MD is seeking a Home Health LPN to deliver 1:1 patient care under the patient’s plan of care. This full-time role offers Monday–Friday scheduling, independence in the home setting, and team support to ensure optimal patient outcomes.

Required qualifications include an LPN/LPT/LVN license, CPR certification, and a valid driver’s license. Home health experience is preferred and benefits are provided with this growing organization.

Qualifications

• Licensed practical nurse with current unrestricted registration in the state of practice
• Valid driver’s license, driving record and proof of car insurance
• CPR certification required
• 1+ year nursing experience preferred
• Home health care experience preferred

Responsibilities

• Provide 1:1 client care following the plan of care with dignity and independence
• Administer medications, feedings, oxygen, ostomy care per plan and assessments
• Document nursing actions and communicate with family, case manager, and physicians
• Coordinate or order supplies, medications, and feedings as needed
• Ensure patient needs align with physician instructions
