Job summary

Adoration Home Health, an affiliate of BrightSpring Health Services, is seeking a dedicated LPN for Wayne, NJ. This full-time role provides 1:1 home health care, following each patient’s plan of care with a focus on dignity and independence.

Candidates should hold an LPN/LPT/LVN license, have CPR certification, a valid driver's license, and ideally 1+ year of nursing and home health experience. Comprehensive benefits offered.

Qualifications

• LPN/LPT/LVN degree with current unrestricted registration and license in the state of practice and residence
• Valid driver's license with proof of car insurance
• CPR certification
• 1+ years nursing experience preferred
• Home health care experience preferred

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Document nursing actions of all care given and communicate with family, case manager, and physicians
• Coordinate ordering of supplies, medications, and feedings to ensure adequate on-hand amounts
• Ensure all patient needs are met per physician instructions
