Job summary

Adoration Health, an Adoration Home Health affiliate, is seeking a Home Health Registered Nurse to join the Wayne, NJ team. You will provide expert, patient-centered care in a home setting, coordinating with MD orders and family caregivers, with on-call rotation as required.

Qualifications

• One year nursing practice in a patient care setting is required
• Valid CPR certification is required
• Current unrestricted RN license in the state of practice and residence is required
• New nursing graduates may be considered in select markets based on program availability

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans that align with MD orders and patient goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with an interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by the local branch
