Adoration Health, an affiliate of BrightSpring Health Services, seeks a dedicated Home Health Licensed Practical Nurse to join our team in Hagerstown, MD. This full-time position offers 1:1 patient care in the home and a supportive environment with opportunities for growth.

Responsibilities include medication administration, documentation, and coordination of supplies, with flexible scheduling and a comprehensive benefits package.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in state of practice and residence
• Valid driver’s license with proof of insurance and safe driving record
• CPR certification required
• 1+ years of nursing experience preferred, home health care preferred

Responsibilities

• Provide 1:1 home health care following the client’s plan of care while maintaining dignity
• Administer medications, feedings, oxygen, and ostomy care as directed
• Document nursing actions and communicate with family, case manager, and physicians
• Coordinate supplies, medications, and equipment to ensure availability
• Ensure patient needs are met per physician instructions
