A healthcare organization in Singapore is seeking a dedicated Hospice/Palliative Care Nurse to provide personalized and compassionate care to patients facing advanced illness. In this role, you will conduct home visits, prepare care plans, and collaborate with a multi-disciplinary team to ensure holistic support for patients and families. The ideal candidate will be a registered nurse with a strong foundation in palliative care, excellent communication skills, and a sincere passion for supporting those in their final stages of life.

Qualificações

  • Registered nurse with at least 3 years of relevant experience.
  • Experience in palliative or geriatric care is an advantage.
  • Passion for hospice and palliative care.

Responsabilidades

  • Conduct home visits and perform night call duties when scheduled.
  • Prepare and write care plans for new patients.
  • Work in collaboration with a multi-disciplinary team.
