Responsibilities

• Review and coordinate individual care plans with stakeholders.
• Support referrals and admissions, including intake briefings.
• Accompany residents to clinics and home visits.
• Build relationships with prospective employers and partner agencies.
• Respond to crises with calm, clinical judgement.
• Maintain accurate case records and progress notes for audits.
• Prepare reports for MSF and social referrals to high standard.
• Maintain regular communication with volunteers and partner agencies.
• Assist with data entry and updating resident information.
• Undertake other duties as directed by supervisor.
