Job summary

NTUC Health Co-operative Limited (NTUC Health) in Singapore is seeking a Senior Social Worker to deliver comprehensive psychosocial assessment, intervention, and case management for clients with complex needs within the community. You will support clients and caregivers to age-in-place with dignity through coordination of care and advocacy.

Role requires leadership in supervision, collaboration with multidisciplinary teams, and strong documentation for service delivery.

Qualifications

• Degree or postgraduate qualification in Social Work recognised by SASW.
• Registered Social Worker (RSW) preferred.
• Minimum 5–8 years of relevant social work experience, preferably in healthcare/e eldercare.
• Experience managing complex cases and working with vulnerable populations.
• Supervisory or mentoring experience preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments for clients and caregivers.
• Develop and implement person-centred care plans based on clients' needs and goals.
• Provide counselling, emotional support, crisis intervention and practical assistance to clients and families.
• Manage complex and high-risk cases independently.
• Monitor clients' progress and review intervention plans regularly.
• Facilitate referrals and coordinate services with hospitals, community partners, government agencies and healthcare providers.
