This role supports case management and administrative functions within a therapeutic team serving residents in care. You will work alongside social workers and clinical staff to coordinate care plans, manage documentation, and respond to resident needs with clinical judgement and compassion.
Review individual care plans with relevant stakeholders and participate in case management discussions with MSF
Support the Therapeutic Team in referral and admission processes, including assisting in intake briefings for new admissions
Accompany residents to clinic or medical appointments and conduct home visits
Build relationships with prospective employers
Respond to crises — self-harm, cutting, suicidality, acute dysregulation, aggression — with calm and sound clinical judgement
Maintain accurate, timely case records and progress records of residents in line with audit and compliance requirements by MSF
Prepare and vet reports for timely submission to MSF and the Court, and social reports for referrals to a high standard
Maintain regular communication with volunteers and partnering agencies to ensure clear communication of programme processes
Assist with data entry, including input and update of resident information
Undertake any other duties as directed by the Senior/Assistant Senior Social Worker and/or Head of Centre
Degree or Diploma in social work
Healthy boundary management
Interest in working with trauma
Knowledge and application of the SASW code of professional ethics and Code of Social Work Practice
Willing to be supervised and open to feedback for change
Strong communication skills (written and spoken)
Team player
Proficient with MS applications
