PPIS Family Services Ltd. in Singapore is seeking a committed social work professional to handle casework, counselling and community-based programmes. You will assess needs, plan interventions and collaborate with agencies to support clients, while maintaining accurate electronic case recordings.

The role involves coordinating with internal teams, planning group activities and ensuring high standards of client service and professional conduct at all times.

Formación

• Degree or Post-Graduate Diploma in Social Work.
• Min 3-5 years relevant work experience in Social Work setting.
• Ability to handle work professionally, independently and efficiently with a strong customer service mindset.
• Effective organizational and follow-up skills, paying attention to detail.
• Good interpersonal skills to interface with all levels, professionally and responsively.

Responsabilidades

• Maintain at least 80% of active cases per quarter of which 100% to be supported with electronic recordings.
• Conduct assessment, planning and intervention for cases.
• Participate in internal and external case conferences, networking and case management meetings.
• Document and evaluate case outcomes.
• Develop and review appropriate intervention strategies.
• Adhere to CSWP and SSNet processes in managing cases.
• Provide accurate information and refer clients to appropriate agencies for further intervention.
• Attends to walk-in clients.
• Adhere to CSWP and SSNet processes in managing referral.
• To provide support to Coordinator in planning, implementation, evaluation and documentation of the programme.
• To plan and organise events and activities for clients.
