Qualifications

• Understanding of social work values and ethics.
• Understanding of work with vulnerable populations (e.g., low-income families).

Responsibilities

• Manage 25-30 C&C active cases per quarter (low-moderate complexity).
• Conduct assessment, planning and intervention with information from practice guides and frameworks.
• Conduct case conferences with partners and participate in meetings.
• Document and evaluate case outcomes and ensure timely reports.
