Job summary

Thye Hua Kwan Moral Charities is seeking a dedicated social worker to conduct case management, psychoeducation, and group work for residents at THK Homes for Disabled in Singapore. The role involves assessments, home visits, and coordinating with families and the multidisciplinary team to ensure holistic care.

The candidate should hold a recognised degree in social work, possess strong communication and documentation skills, and demonstrate discretion when handling confidential information.

Qualifications

• Minimum Degree or Post Graduate Qualifications in Social Work from a recognised institution.
• Good listening and communication skills to communicate with clients with diverse abilities and backgrounds.
• Good written communication skills to document information in a clear, concise, and accurate manner.
• Good verbal and interpersonal skills to liaise with stakeholders and external agencies.
• Ability to exercise good judgement and discretion in dealing with confidential information.

Responsibilities

• Conduct case management and psychoeducation for residents.
• Plan and facilitate group work for residents at THK Homes for Disabled.
• Manage enquiries and referrals.
• Conduct assessments and home/site visits; provide counselling and behavioural interventions.
• Support in the submission of HOMES means testing and transport subsidy applications.
• Coordinate with multidisciplinary team for crisis management and care plans.
• Maintain accurate case records and prepare reports for governing bodies.
