Care Corner Singapore Ltd is seeking a dedicated Social Worker to manage caseloads, assess needs, plan interventions, and collaborate with partners to support vulnerable populations in the community.

The role involves case conferences, networking, documentation, and timely reporting, with opportunities to contribute to program development and community outreach. Fresh graduates are welcome to apply and internships are considered.

Qualificações

• Degree or Graduate Diploma in Social Work (mandatory)
• Fresh graduates in Social Work are welcome to apply; internships/placements will be considered
• Social Work education and understanding of social work values and ethics
• Understanding of work with vulnerable populations i.e. low-income families

Responsabilidades

• Manage 30 C&C active cases (mainly Group 2 cases) per quarter.
• Conduct assessment, planning and intervention for cases.
• Conduct case conferences with partners.
• Participate in networking and case management meetings.
• Document and evaluate case outcomes.
• Develop and review appropriate intervention strategies.
• Ensure reports on cases are submitted in a timely manner.
