Qualifications

• Minimum degree in Social Work or related field with internship experience.
• Experience with case management and coordinating care services.
• Ability to engage clients and build rapport.

Responsabilités

• In consultation with the program lead, conduct assessment, planning, and intervention for cases.
• Engage clients and build rapport.
• Develop, implement and review individual care plans and intervention strategies.
• Perform needs assessment of clients and caregivers.
• Advocate for clients’ needs.
• Coordinate services with other community resources on the client’s behalf.
• Case documentation.
• Manage case referrals.
• Conduct and participate in case conferences/MDT meetings.
• Monitor delivery of coordinated services.
• Document and evaluate case outcomes.
• Adopt a case management approach when working with clients.
• Collaborate with internal business units and other agencies/specialists to provide multi-disciplinary assessments and monitoring of clients.
