Care Corner Singapore Ltd. is seeking a social work professional for a role focused on case management, evaluation, and collaboration with partner agencies in Singapore. The position involves handling multiple cases, conducting intake assessments, and contributing to community outreach initiatives.

The role emphasizes documentation, adherence to social work practice standards, and teamwork within an intake/triage and group-work framework.

Competenze

• Degree or Graduate Diploma in Social Work or related field such as Psychology, and other relevant fields in Social Science
• Previous experiences via internships or placements in related areas of work will be considered
• Understanding of social work values and ethics
• Understanding of work with vulnerable populations i.e. low-income families

Mansioni

• CASEWORK AND COUNSELLING: Manage 25 active cases per quarter, mostly low to moderate complexity; conduct assessment, planning and intervention with information from practice guides and organizational frameworks; conduct case conferences with partners; participate in networking and case management meetings; document and evaluate case outcomes; develop and review intervention strategies; ensure proper documentation and timely reports.
• INFORMATION AND TRIAGE: Manage case referrals; participate in Intake & Triage Team to conduct intake assessments based on Code of Social Work Practice and Practice Guide; refer clients to appropriate agencies for intervention.
• COMMUNITY WORK: Network with other organizations and stakeholders; participate in implementing and evaluating community outreach/initiatives.
• GROUP WORK: Be part of the programme team for Group Work conducted at the FSC.
