Qualifications

• Degree or Post-Graduate Diploma in Social Work.
• 3–5 years relevant work experience in social work.
• Ability to handle work professionally, independently and efficiently with a strong customer service mindset.
• Possess effective organizational and follow-up skills, paying attention to detail.
• Good interpersonal skills and able to interface with all levels, internally and externally, in a professional and responsive manner.

Responsibilities

• Maintain at least 80% of active cases per quarter with 100% supported by electronic recordings.
• Conduct assessment, planning and intervention for cases.
• Participate in internal and external case conferences, networking and case management meetings.
• Document and evaluate case outcomes.
• Develop and review appropriate intervention strategies.
• Adhere to CSWP and SSNet processes in managing cases.
• Provide accurate information and refer clients to appropriate agencies for further intervention.
• Attend to walk-in clients.
• Adhere to CSWP and SSNet processes in managing referrals.
• Provide support to Coordinator in planning, implementation, evaluation and documentation of the programme.
• Plan and organize events and activities for clients.
