Job summary

DEMENTIA SINGAPORE LTD. seeks a committed dementia care coordinator to assess cases and develop individualized care plans for persons with dementia and their carers. The role includes planning and delivering group sessions, providing psychoeducation, and linking services with partners.

Qualifications

• Minimum 2 years of experience in community engagement and/or case management.
• Good oral, written and interpersonal communication skills.
• Ability to conduct Advance Care Planning conversations or prior experience with persons with dementia or carers is an added advantage.
• Willing to work on Saturdays and occasional weekday evenings.
• Ability to facilitate group sessions will be an added advantage.
• Self-motivated with a high level of initiative.
• Resourceful and adaptable.
• Strong attention to detail.
• Good organizing and execution skills.

• Explore new avenues of collaboration with community partners and stakeholders.
• Support in outreach events and talks.
• Any ad-hoc tasks as and when required.
