Job summary

Celecti Pte Ltd is seeking Social Workers and Senior Social Workers to join their growing team. You will conduct psychosocial assessments and develop person-centred care plans for clients and caregivers, while coordinating with internal teams and community partners.

Responsibilities include counselling, case management and supporting advance care planning, with travel to home visits and different sites. Senior level guidance will be provided where applicable.

Qualifications

• Postgraduate degree in Social Work recognised by SASW.
• Registered Social Worker (RSW) with SASW.
• Min. 2 years of community or elder care social work experience.
• Willing to travel for home visits and sites; travel allowance provided.
• Experience in palliative care is an advantage.

Responsibilities

• Conduct psychosocial assessments and develop person-centred care plans for clients and caregivers.
• Assess, care plan and intervene for seniors and caregivers.
• Provide counselling, psychosocial support and advocate for clients’ needs.
• Manage case referrals and participate in case meetings.
• Coordinate and monitor services with internal teams and community partners.
• Provide information on assistance schemes and care options to support informed decisions.
• Support advance care planning (ACP) and end-of-life discussions where relevant.
• Maintain accurate case records and progress reports in a timely manner.
• Provide clinical supervision and guidance to junior staff (Senior Social Worker level).
