Job summary

Celecti Pte Ltd is seeking a dedicated Social Worker to join our team in Singapore. You will manage case referrals, collaborate with the centre lead for assessments and interventions, and monitor the delivery of coordinated services to clients.

The role requires a Bachelor's in Social Work (or related fields) and at least 1 year of experience in social work, preferably in community or healthcare settings. Strong communication and stakeholder management skills are essential, with proficiency in

Qualifications

Bachelor's degree required, preferably in Social Work.
Min. 1 year of experience in social work, preferably in community or healthcare.
Strong communication and stakeholder management skills; proficient in Google Suite.

Responsibilities

Manage case referrals.
Consult with the centre lead to conduct assessment, planning and intervention for cases.
Monitor the delivery of coordinated services.
Adopt a suitable case management approach when working with clients.
Refer and coordinate care services to ensure clients are adequately supported.
Keep abreast of, and provide information on, the various assistance schemes and care options available.
Assess family financial status.
Manage the administration of grants, subsidies and support services where relevant.
Ensure all MOH service requirements on financial and quality aspects are adhered to.
