Job summary

NTUC Health Co-Operative Ltd invites Social Workers to join our Active Ageing Centre, Senior Day Care and Home Care teams. You will conduct psychosocial assessments, provide counselling for seniors and caregivers, and coordinate care across multiple service lines to promote wellbeing and ageing in place.

The role emphasises person-centred practice, collaborative care planning, and linking clients to community resources. Experience with eldercare and complex cases is valued.

Qualifications

• Degree in Social Work recognised by SASW.
• RSW preferred or eligible for registration.
• 2–5 years of experience in community care, eldercare, medical social work, home care or social service settings.
• Experience with frail older adults and caregivers is preferred.
• Knowledge of Singapore's community care ecosystem and long‑term care services.

Responsibilities

• Conduct psychosocial assessments and care planning across AAC, SDC and Home Care.
• Provide counselling and caregiver support, crisis intervention and related social work interventions.
• Coordinate care across internal services, clinicians and community partners.
• Maintain documentation and participate in quality improvement and service development initiatives.
• Support care transitions and ensure continuity of care for seniors and caregivers.
