Job summary

In this role, you will actively contribute to departmental quality improvement initiatives and work within eldercare settings, requiring professional SASW registration and 3+ years of relevant experience.

Qualifications

• Master/Bachelor/Post Graduate Diploma in Social Work.
• Registered Social Worker with the Singapore Association of Social Workers (SASW).
• Minimum 3 years of relevant working experience in Social Work and eldercare setting.
• Family Service Centre (FSC) experience is required.

Responsibilities

• Conduct psychosocial and financial assessments.
• Provide direct counselling, emotional support, and psychoeducation.
• Act as liaison for patients/clients and their families.
• Maintain accurate, comprehensive, and timely documentation of assessments, interventions, care plans.
• Actively participate in and contribute to departmental and organizational quality improvement initiatives.
