Job summary

Recruit Express Pte Ltd is seeking a Social Worker to conduct psychosocial assessments, provide counselling, emotional support and develop care plans for clients and families. You will coordinate referrals to healthcare, social services and community resources, work with multidisciplinary teams and support caregivers to ensure continuity of care.

A recognised degree or postgraduate diploma in Social Work is required; strong communication and interpersonal skills and the ability to manage cases

Qualifications

• Degree or Postgraduate Diploma in Social Work from a recognised institution.
• Experience in healthcare, community care or social services is advantageous.
• Knowledge of community resources and social assistance schemes.
• Strong assessment, counselling and case-management skills.
• Good communication and interpersonal skills.
• Able to work with clients, families, multidisciplinary teams and community partners.
• Empathetic, organised and able to manage cases independently.

Responsibilities

• Conduct psychosocial assessments to identify client and family needs.
• Provide counselling, emotional support and social interventions.
• Develop individualised care plans based on identified needs.
• Provide case management and follow-up to ensure continuity of care.
• Assist clients and families in accessing financial assistance and services.
• Coordinate referrals to healthcare, social services and community agencies.
• Collaborate with multidisciplinary teams and external partners.
• Support caregivers and family members.
• Maintain accurate case documentation and records.
• Participate in case conferences and care planning.
• Support programmes and outreach activities as required.
