Job summary

ACTIVE GLOBAL RESPITE CARE PTE. LTD. is seeking a dedicated Home Care Coordinator to screen referrals, review care needs, and ensure services follow care plans during home visits across Singapore.

You will conduct in-home and hospital assessments, coordinate with doctors, nurses, therapists, MSWs and HCAs, raise referrals via BRIGHT, assist with household means testing, and support discharge planning.

Responsibilities

• Screen and review all ISC referrals for medical, functional, and cognitive conditions, and identify the client's care needs if the referral source has not already done so.
• Ensure ISC care staff provide care according to the referral source's care plan and the client's needs.
• Follow-up with ISC clients midway through the service as and when required to ensure they have a longterm care plan.
• Conduct home and hospital visits for potential clients; these visits will account for about 70% of the time.
• Where necessary and applicable, refer the client and/or caregiver to relevant services or healthcare providers.
• Raise referrals for eligible clients via BRIGHT to required services; submit appeals or extensions where applicable.
• Assess and submit Household Means Testing application for clients.
• Conduct interRAI assessment and develop an individualised care plan.
• Work with referral sources on the client's discharge care plan.
• Coordinate client care with doctors, nurses, therapists, MSWs, and HCAs within the company.
• Advise clients on financial scheme applications.
• Handle client and referral source inquiries about the service.
• Assist with audit-related tasks when required.
• Handle any ad hoc tasks assigned by the Head of Home Care.
