Job summary

The Supreme HR Advisory Pte. Ltd. is seeking a focused coordinator to manage hospital referrals and start-of-care processes. You will handle scheduling for Nursing Aide/Caregiver staff, maintain daily attendance, and liaise with clients, families, hospitals, and the IC team to ensure smooth service delivery.

Based in Tagore Lane, this role requires coordinating last-minute changes, with a Diploma or relevant healthcare background, and a readiness to assist with administrative tasks as needed.

Qualifications

• Diploma or relevant experience in Healthcare, Nursing, Community Care, Social Services or Healthcare Administration
• Comfortable coordinating staff schedules and handling last-minute changes

Responsibilities

• Coordinate and manage IC referrals from hospitals
• Contact clients and families to arrange the start of care services
• Coordinate Nursing Aide/Caregiver assignments and schedules
• Monitor daily attendance and ongoing client services
• Arrange replacement staff when necessary
• Communicate regularly with clients, families, hospitals and the IC team
• Monitor clients' service periods and upcoming discharge dates
• Assist on simple admin task
