Job Description

The Senior Executive / Assistant Manager, Population Health and Integrated Care (PHIC) - Integrated Continuing Care (ICC), will play an integral role in the team in overseeing and managing a spectrum of Population Health programs. He/she will work with the ICC team, in partnership with various stakeholders, in both operation and strategic planning aspects of the programs.

Your job roles will include, but not be limited to the following:

  • Responsible for daily operations of the programme
    • Oversee and play a key role in operations processes
    • Identify key areas of change and develop new workflows/adjusting existing workflows where necessary to cater for programme/landscape changes where necessary
    • Problem-solving when adhoc situation arises
  • Regular maintenance and improvement of databases to keep an accurate and updated record of cases that are received by the programme.
  • Oversee all inventory matters for the programme.
  • Collate and report data to internal/external stakeholders (including SingHealth Office of Regional Health, SORH) at regular intervals where needed.
  • Facilitate, support and conduct presentations (including preparation of slides and taking minutes) at regular meetings including administrative; programme, department and division level meetings.
  • Any other adhoc duties as assigned.
Job Requirements
  • General Bachelor's Degree
  • Proficiency in Microsoft Office Applications, particularly Microsoft Excel spreadsheets and application of formulas
  • Relevant working experience of at least 2-3 years in running operations within healthcare or other industries is preferred
  • Strong oral and written communication skills
  • Resourceful and meticulous
