Job summary

NTUC Health Co-operative Limited is seeking a Care Navigation team member to deliver customer service excellence for seniors and families, applying empathy and understanding to recommend appropriate eldercare services.

You will handle calls and emails, assess needs, facilitate enrolment, conduct needs assessments with seniors and families, coordinate with service providers, attend meetings and prepare reports for regulators and stakeholders, and may represent NTUC Health at roadshows.

Qualifications

• Minimum 2–3 years of relevant experience.
• Diploma or degree in any discipline.
• Healthcare or community care experience preferred.
• Strong communication, patient, meticulous and tech-savvy.

Responsibilities

• Attend customers' calls and email inquiries.
• Assess customers' needs and recommend services.
• Facilitate enrollment into services.
• Conduct needs assessments with seniors and families.
• Collaborate with service providers for enrolment and ongoing support.
• Attend meetings, take minutes, and track follow-ups.
• Prepare reports for regulators and stakeholders.
• Represent NTUC Health at roadshows to promote eldercare services.
