Job summary

NTUC Health Co-operative Limited is seeking a dedicated Community Care Coordinator to support home care services in Singapore. You will coordinate information, referrals, and case management to ensure high-quality client outcomes and satisfaction for elderly clients and their families.

You will conduct regular home visits and follow-ups, collaborate with healthcare partners, and help with service development and process improvements to meet evolving eldercare needs.

Qualifications

• Diploma or Advanced Diploma in a healthcare-related discipline.
• At least three years of experience in community healthcare settings in Singapore.
• Proficient in Google Workspace and Microsoft Office applications.
• Demonstrate good customer service and communication skills.
• Fluent in English and at least one additional local language.
• Strong team player with ability to multitask.
• Enjoy interacting with people and collaborating within teams.
• Be able to work independently and meet deadlines; passion for community service.

Responsibilities

• Manage walk-in, online and phone enquiries on NTUC Health services.
• Understand needs of customers using screening tools and promote suitable NTUC Health services.
• Collaborate with NTUC Health divisions and external partners on service referrals and ensure follow-up on action points.
