NTUC Health Co-operative Limited is seeking a Senior Care Coordinator (1 Year Contract) to provide client-centric care coordination, discharge planning, and ongoing case management for Home Care services in Singapore.

You will conduct home visits, coordinate service referrals, and collaborate with internal teams and external partners to ensure timely and quality care for elderly clients and their families.

Qualifications

  • Diploma or Advanced Diploma in a healthcare-related discipline.
  • At least three years of experience in community healthcare settings in Singapore.
  • Proficient in Google Workspace and Microsoft Office applications.
  • Demonstrate good customer service and communication skills with internal and external stakeholders.
  • Fluent in English and at least one additional local language.

Responsibilities

  • Information and Referral (I & R): Manage walk-in, online and phone enquiries.
  • Service Coordination: Administer coordination of services requested by clients and document promptly.
  • Case Management: Conduct regular home visits and follow-ups with clients and caregivers.
  • Service Development & Revenue Growth: Participate in programme development and assist with initiatives to grow service utilisation.

Job Description

To provide client-centric care coordination, discharge planning, monitoring/evaluation and education for clients and their families on Home Care Services offered by NTUC Health Co-Operative Ltd.

You will need to:

(a) Information and Referral (I & R)

  • Manage walk-in, online and phone enquiries on NTUC Health services.
  • Understand the needs of customers through the use of screening tools, and promote a suitable package of NTUC Health services to customers.
  • Collaborate with NTUC Health divisions and external partners on service referrals, submit referral forms and ensure follow-up on action points.

(b) Service Coordination

  • Administer the coordination of services requested by clients.
  • Ensure proper documentation on the systems and paper filing are completed on a timely basis.
  • Work closely with internal stakeholders when applicable to ensure the smooth delivery of service, i.e. Home Personal Care (HPC) service is carried out by Care Associate.
  • Assist in appointment scheduling for various Home Care services.

(c)Case Management

  • Conduct regular home visits and telephonic follow-ups on a monthly basis to engage and build rapport with clients and their caregivers, and obtain feedback on the services rendered.
  • Collaborate with other social and health partners to recommend care programs according to the needs of clients, and assist with appeals submission when applicable.
  • Counsel and educate caregivers on providing appropriate care, support and financial assistance.
  • Arrange for a reassessment of care plans with clients on a 6-monthly basis. This is to determine if the client is required to continue to receive the service.
  • Work closely with the Operations and Nurse Manager on discharging clients from service.

(d)Service Development

  • Participate in regular programme development and evaluation in order to meet the needs of an evolving eldercare sector.

(e)Revenue Growth & Service Utilisation

  • Support in initiatives to drive revenue growth and enhance service utilisation by identifying opportunities, optimising resources, and implementing strategies to achieve business KPIs and maximise client satisfaction and impact within the zone(s).

Qualification

You should:

  • Possess a Diploma or Advanced Diploma in a healthcare-related discipline
  • Have at least three years of experience in community healthcare settings in Singapore
  • Be proficient in Google Workspace and Microsoft Office applications
  • Demonstrate good customer service and communication skills, with the ability to engage effectively with both internal and external stakeholder
  • Be fluent in English and at least one additional local language to communicate effectively with elderly clients and their families
  • Be a strong team player with the ability to multitask
  • Enjoy interacting with people and collaborating within teams
  • Be able to work independently and meet deadlines
  • Have a passion for community service and a service-oriented mindset
Other Information

It would be great if you:

  • Have experience working in the eldercare/healthcare/community care or client facing sectors
  • Have the ability to converse in local dialects to facilitate effective communication with elderly clients and their next-of-kin
