Job summary

Recruit 123 is partnering with a respected medical center in western Singapore to recruit a Care Coordinator for community healthcare. You will support seniors, coordinate care across medical, nursing, and rehabilitative services, and collaborate with doctors, nurses, and allied health professionals to enhance patient outcomes.

We seek a Registered Nurse with SNB registration and 3–5 years of psychiatric experience, ideally in community clinics or IMH.

Qualifications

• Registered Nurse with SNB registration.
• 3–5 years of psychiatric experience, preferably in community clinic or IMH.
• Strong organizational, interpersonal, communication and writing skills.
• Proficient in MS Office and healthcare environments.

Responsibilities

• Conduct home visits to provide psycho-education, support, and care coordination for clients and caregivers.
• Assess patients' medical, psychological, social, environmental, and vocational needs.
• Collaborate with Allied Health Professionals, doctors, and nurses to ensure holistic care.
• Provide psychotherapy insights in multidisciplinary meetings.
• Facilitate mental health support groups for patients and caregivers.

Key Responsibilities:

• Conduct home visits to provide psycho-education, support, and care coordination for clients and caregivers
• Assess patients' medical, psychological, social, environmental, and vocational needs
• Collaborate with Allied Health Professionals, doctors, and nurses to ensure holistic care
• Offer psychological expertise in multi-disciplinary meetings
• Provide training for team members and caregivers
• Facilitate Mental Health support groups for patients and caregivers
• Execute projects and responsibilities assigned by the supervisor or Head of Department

Requirements:

• Registered Nurse with the Singapore Nursing Board
• RMN/Advanced Diploma in Mental Health or relevant qualifications (e.g., social work, counseling, psychology)
• 3 to 5 years of psychiatric experience (Community psychiatric clinic/IMH preferred)
• Skilled in psychotherapy for mental health patients
• Strong organizational, interpersonal, communication, and writing skills
• Proficient in MS Office and familiar with healthcare environments
• Ability to multi-task, work independently, and collaborate effectively
• Passionate about supporting individuals with mental health disorders
