Job summary

Agency for Integrated Care in Singapore seeks a Programme Manager for the Community Mental Health Master Plan Programmes (ASCAT and Aftercare). You will ensure programmes operate according to the model of care and objectives, coordinating with partners to meet KPI targets and deliverables.

You will analyse service gaps, design new care models, and work with Restructured Hospitals and Regional Healthcare Systems to translate data into actionable plans, drive improvement, and strengthen population

Qualifications

Bachelor's degree in social work, psychology, counselling or related field.
Minimum 3 years' relevant experience with multi-agency work and programme management.
Experience in programme evaluation, design thinking and organisation development.
Experience in community development within healthcare or social care.
Ability to work well in a team, manage change, and handle multiple tasks.

Responsibilities

Analyse service gaps and develop new models of care to augment community services.
Collaborate with Restructured Hospitals and Regional Healthcare Systems to align outcomes and plans.
Work with providers on KPI submissions, claims processing and reporting.
Translate data and programme insights into actionable plans to improve effectiveness.
Develop capacity of partners in line with tiered care model competencies.
