Job summary

NATIONAL UNIVERSITY HOSPITAL (SINGAPORE) PTE LTD is seeking a coordinator for a community-based preventive paediatrics health program. You will coordinate care across preschools, social workers, and the Child Development Unit, and support data collection and multidisciplinary collaboration.

Responsibilities include home visits, screening and assessing child development and health, and guiding families through clinical pathways.

Qualifications

• A Bachelor’s degree in Nursing, Psychology, Behavioural Science, Social work, Health Sciences, or Education.
• Excellent command of spoken and written English, with a second language.
• Strong interpersonal and communication skills; able to work in a team or independently.

Responsibilities

• Coordinate care for children and families in a community-based preventive paediatrics program.
• Manage patients on preventive programs with screening and health education.
• Assist in data collection and database management (RedCAP/ Excel).
• Screen, assess and identify medical, psychological, social, environmental, and financial needs.
• Direct patients/families to hospital or partner programs per clinical protocol.
• Collaborate with clinicians and teams to implement and monitor care plans.
• Conduct home or community visits as needed for care coordination.
• Participate in workgroup discussions and report program statistics.
