Job summary

National University Health System Pte. Ltd. seeks a Care Coordinator to support case management and care coordination across Alexandra Hospital’s programmes. You will liaise with Clinical Directors, programme leads and the care team to optimise patient journeys within NUHS channels.

You will collaborate with internal and external partners to review models, coordinate referrals and ensure timely follow-up, contributing to improved health outcomes and continuity of care for patients and caregivers.

Qualifications

• Degree or equivalent professional qualifications in Nursing, Social Work or Allied Health.
• Prefer nursing, case management or related field experience in acute/community settings in Singapore.
• Minimally 5 years of healthcare industry experience.
• Strong teamwork and interpersonal skills with clinicians and care teams.

Responsibilities

• Execute and review care models and workflows with supervisors and counterparts.
• Advocate for case management and care coordination as integral to hospital objectives.
• Coordinate with Ops and other stakeholders to improve programmes and patient flow.
• Ensure documentation, accountability and continuity across care transitions.
