Job summary

National University Health System is seeking an energetic Health Promotion/Care Coordinator to support mother-child dyads under the ChAMPION program in Singapore. You will screen, assess, provide anticipatory guidance and psychoeducation aligned with milestones, and coordinate care through protocol-driven referrals.

You will also maintain recruitment records, liaise with Family Nexus coordinators, and collaborate with Research Assistants on data collection and surveys.

Qualifications

• Diploma in Health Promotion or Sciences preferred.
• Basic healthcare experience advantageous.
• Proficient in MS Office applications and IT navigation.
• Strong communication and coordination skills.
• Proactive and a team player.

Responsibilities

• Ensure completion of screening, assessment, anticipatory guidance and psychoeducation for mother-child dyads according to milestones.
• Assist in care management and cross-referrals to specialists through dedicated protocols, based on mother and child profile and needs.
• Maintain a record of mother-child dyads/parents recruited in ChAMPION program and other data collation accurately on a timely basis.
• Work with Family Nexus care coordinators to assist with arrangement of appointments as required.
• Work cohesively with Research Assistants in data collection and surveys.
• Any other responsibilities as assigned by the Reporting Officer
