You will coordinate case management, liaise with stakeholders, document workflows, and contribute to quality improvement while empowering patients and caregivers to navigate the community care continuum.

Qualifications

• Degree or equivalent professional qualifications in Nursing, Social Work or Allied Health.
• Experience in healthcare settings in Singapore preferred.
• Minimally 5 years of work experience in healthcare industry.
• Strong communication and interpersonal skills.

Responsibilities

• Coordinate care management and liaise with clinical programmes and service leads.
• Advocate for case management and care coordination.
• Collaborate with internal and external stakeholders to improve patient care.
• Ensure accurate documentation and smooth care transitions.
• Support and supervise Care Coordinators and associates.
