Job summary

Responsibilities

Coordinate care for patients and families in a community-based preventive paediatrics health program across community sites (preschools), social workers and the Child Development Unit.
Manage patients on preventive program with screening and health education; ensure data collection and clinical visits follow through.
Assist in ethics submission and data collection; maintain database in RedCAP or Excel; manage consent procedures for participation in research.
Participate in workgroup meetings and program teachings; provide statistical and program reports when required.
