Job summary

Montfort Care in Singapore is seeking an Assistant Nurse Manager (HPC & HPC+) to support the Nurse Manager in clinical leadership, governance and operational oversight for Home Personal Care services. The role blends hands-on community nursing with team supervision and service coordination to ensure safe, person-centred care for clients with complex needs.

Qualifications

• Bachelor's degree in Nursing and SNB registration.
• Post-basic qualification in community nursing, gerontology, palliative care or leadership preferred.
• 4–6 years of nursing experience, with community/home care exposure.
• Senior staff nurse/clinical supervision experience is advantageous.

Responsibilities

• Provide direct nursing care and clinical oversight for HPC/HPC+ clients.
• Support care planning, clinical decision-making and escalation per protocols.
• Monitor outcomes to ensure safety and continuity of care in the community.
• Ensure accurate clinical documentation and records.
• Supervise nursing staff and mentor care personnel on site.
