Job summary

ScienTec Consulting Pte. Ltd. in Singapore (Kent Ridge) is seeking a permanent role to support daily operations of Community Care Teams, coordinate with community partners, and monitor KPIs and performance.

The position also involves implementing new programmes, process improvements and technology solutions. Applicants should hold a Bachelor’s Degree and have at least 2 years of relevant experience; healthcare, operations or customer service exposure is advantageous.

• Minimum 2 years of relevant working experience.

Responsibilities

• Support daily operations of Community Care Teams, including transition care programmes and Community Health Posts.
• Engage and coordinate with community partners and stakeholders.
• Monitor operational KPIs, workload and performance.
• Support implementation of new programmes, process improvements and technology solutions.
• Manage residents' feedback, complaints and service recovery.
• Identify service gaps and recommend improvements to enhance patient and resident experience.
