Qualifications

Responsibilities

• Coordinate support to partner organisations to ensure clear roles and workflows.
• Facilitate group lead meetings and track actions/outcomes.
• Identify and resolve gaps in service delivery and improve collaboration.
• Assess resources and opportunities for pooled efforts across ICCP partners.
• Coordinate standardised care assessment tools like InterRAI.
• Assist in developing shared care plans for seniors.
• Support MOH and AIC reporting and funding applications.
