Job summary

Home Nursing Foundation seeks an Executive to coordinate ICCP programme activities and drive care coordination for elderly seniors across our sub-regions in Singapore. You will liaise with network partners, support governance meetings, and help align services to organisational goals.

The role emphasizes strong stakeholder management, InterRAI assessments, and monitoring of service delivery to identify improvements. You will report to the Head of Regional Services in a fast-paced environment.

Qualifications

• Degree in social work, nursing or healthcare-related discipline would be an advantage.
• Strong interpersonal communication and stakeholder management skills are essential.
• Experience coordinating integrated care initiatives is a plus.

Responsibilities

• Implement ICCP programme activities and workplans, piloting joint referral workflows and care coordination initiatives.
• Conduct InterRAI care assessments and undertake care planning and coordination responsibilities.
• Serve as primary liaison with partner organisations to enable service integration across sub-regions.
• Represent HNF in ICCP sub-regions, aligning with organisational objectives and partnerships.
• Support community outreach to raise ICCP awareness and collaboration with stakeholders.
• Organise ICCP Steering Committee and operational meetings, prepare agendas, take minutes, and follow up actions.
• Act as a key liaison between internal teams and external partners for clear communication and smooth execution.
• Monitor service delivery performance and identify improvement opportunities.
• Assist in preparing reports, dashboards, and presentations for management and external reporting.

Programme Implementation & Care Coordination

• Implement ICCP programme activities and workplans, including piloting joint referral workflows and care coordination initiatives to support integrated care delivery.

• Conduct interRAI care assessments and undertake care planning and coordination responsibilities, as assigned.

Partnership Management & Stakeholder Engagement

• Serve as the primary point of contact for partner organisations within the assigned sub-region(s), building and maintaining strong working relationships to facilitate service integration.

• Maintain a strong understanding of HNF services and offerings, and proactively identify opportunities to bridge and align services across participating sub-regions.

• Represent HNF in ICCP sub-regions, ensuring alignment with organisational objectives and fostering effective partnerships.

• Support community outreach efforts to build and strengthen partnerships, raise awareness of ICCP initiatives, and facilitate collaboration with community stakeholders in the sub‑region.

Governance, Coordination & Communication

• Organise and support ICCP Steering Committee and operational meetings, including agenda preparation, minute‑taking, performance reviews, and follow‑up actions to drive strategic and operational alignment.

• Act as a key liaison between internal teams and external partners to ensure clear communication, effective collaboration, and smooth execution of joint initiatives.

Performance Monitoring & Service Improvement

• Monitor service delivery performance, track progress against targets, and support the resolution of operational challenges.

• Identify service gaps and opportunities for improvement, and support the implementation of best practices to enhance service quality across partner organisations.

Reporting & Management Support

• Assist in the preparation of reports, dashboards, and presentations for internal management review and external reporting, as required.

• Support HNF management in maintaining oversight of ICCP sub‑regional involvement, including tracking developments, milestones, and status updates.

Other Information

HNF is an appointedIntegrated Community Care Provider (ICCP) to coordinate care and deliver a suite of baseline services to assigned sub‑regions in Singapore. These services include activities to support active ageing, day care, rehabilitation and support for Activities of Daily Living(ADL)/Instrumental Activities of Daily Living (iADL), typically operated by Active Ageing Centre (AAC), Senior Care Centre (SCC), Enhanced Home Personal Care (HPC+) and Home Therapy (HT) Providers.

HNF is seeking for an Executive who possesses strong interpersonal communication and stakeholder management skills to coordinate these care needs for the elderly seniors in our sub‑regions with our network of providers available in the sub‑region. The incumbent will be reporting to the Head of Regional Services and is expected to able to work effectively in a fast‑paced environment.
