Job summary

Home Nursing Foundation is seeking an Executive to coordinate ICCP services across assigned sub-regions in Singapore. You will implement care programmes, conduct interRAI assessments, and coordinate with a network of providers to ensure seamless service delivery for elderly clients.

You will engage partner organisations, organize steering meetings, monitor performance, and contribute to reports for management.

Qualifications

• Strong interpersonal communication and stakeholder management skills are required.
• Experience coordinating care across multiple partners is preferred.

Responsibilities

• Implement ICCP programme activities and pilot joint referral workflows.
• Conduct interRAI care assessments and care planning.
• Manage relationships with partner organisations and facilitate service integration.
• Organise ICCP Steering Committee and meet minutes.
• Monitor service delivery performance and identify improvement opportunities.
• Assist in reports and dashboards for management and external reporting.
